Denver Nephrology’s Comprehensive Renal Care Clinic (CRCC) was conceived as a means to better recognize and care for patients with potentially worsening CKD. The Clinic, housed at each of our office sites, focuses on early identification and stratification of CKD patients and their associated illnesses (comorbidities), early intervention to treat these comorbidities, and when necessary, timely preparation for dialysis and/or renal transplantation.
The innovative use of advanced practice nurses enhances patient education, coordination of care with other specialists and the patient’s primary care physician, and care plan development and follow-up.
Our service goal is to narrow the gap between and raise the bar from what the patients and their referring physicians might simply accept as adequate care and what they ideally would like to receive from their nephrologists.
